non-diver companion discount= subtract $390 from the diver price.

SWS TRIP PAYMENT REQUIREMENTS:$3OO per person holds your spot. 2/3rds of your trip cost must be on deposit with SWS on or before March 15, 2012 to continue to hold your spot.. Full trip balance due must be paid to SWS no later than August 13, 2012 . No refunds!
SWS TRIP PACKAGE DOES NOT INCLUDE:
1. Make your own flight arrangements. Get the best deal on-line. SWS trip starts and ends in Cozumel. Once Andy and Lynn make their flight reservations, their itinerary will be posted on this site if you want to get on their flights.

TRIP CANCELATION POLICY:
If you must cancel, all deposit money is non-refundable unless we are able to fill your vacated space. If we are able to fill your vacated spot at the full purchase price, SWS will credit your refund to an in-store account minus a per person $100 cancellation fee.If we are unable to fill your spot on the trip, all money paid toward the trip will be forfeited.
Trip Release of Liability Waiver Requirement: Every person who reserves a spot on a trip through SWS must have a completed TRIP WAIVER form on file at SWS before being allowed on the trip. If you do not know the cost of your trip to put in the "amount" space, contact Scuba World Sacramento. Download, print, fill in the blanks, indicate you have read and understand the waiver by signing and dating the SCUBA WORLD TRIP WAIVER FORM and fax the completed form "attention Lynn" to SWS at fax number: 916-338-0277.
NOTE 1. YOU MUST HAVE A PASSPORT VALID FOR AT LEAST SIX MONTHS BEYOND YOUR RETURN DATE to travel to MEXICO. U.S. Passport applications, renewal forms and information can be found on line at PASSPORT HOME
Note 2: Every diver on this trip is required to have Diver Accident Insurance coverage. If you do not wish to join Divers Alert Network and purchase their Diver Accident Insurance, proof of another plan you have that covers you for an an out-of- the- country accident including medical flight evacuation back to the U.S. or to a better medical facility should that be required is acceptable.
