**On this trip, we will be teaching the PADI Advanced Diver Class, Drift Diving and Underwater Naturalist

$150.00. At time you request the Nitrox package be added to your trip package cost, indicate 32 or 36
mix. SWS will provide the dive operator with a list of people pre-paid and pre-reserved along with your

Dbl Occ: Non-diver = $590 (Must share room with diver) Single Occupancy = $1,310 (Must agree to share if rooms become scarce.)

SWS TRIP PAYMENT REQUIREMENTS: As of January 1, 2010, reservation requests are subject to availability. Full trip payment is due now to hold your space. Final balance owed for those of you who booked early is due in to SWS no later than February 25, 2010. Cash/check payment preferred. Possible processing fee charged if you use a Credit Card to pay for your trip.

Trip Cancellation Policy:
If you must cancel, all deposit money is non-refundable unless we are able to fill your vacated space. If we are able to fill your vacated spot at the full purchase price, SWS will credit your refund to an in-store account minus a per person $100 cancellation fee.If we are unable to fill your spot on the trip, all money paid toward the trip will be forfeited.
Trip Release of Liability Waiver Requirement: Every person who reserves a spot on a trip through SWS must have a completed TRIP WAIVER form on file at SWS before being allowed on the trip. If you do not know the cost of your trip to put in the "amount" space, contact LYNN . Download, print, fill in the blanks, indicate you have read and understand the waiver by signing and dating the

If you do not wish to join Divers Alert Network and purchase their Diver Accident Insurance, proof of another plan you have that covers you for an an out-of- the- country accident including medical flight evacuation back to the U.S. or to a better medical facility should that be required is acceptable. NOTE 3. For peace of mind, SWS strongly recommends you purchase additional trip cancellation/interruption coverage to help defray costs arising from unforeseen events. No one plans an emergency. NOTE 4 Divers Alert Network in addition to selling Diver Accident Insurance coverage also sells trip cancellation and interruption insurance coverage which is an expanded version of the basic DAN diver accident coverage. SWS highly recommends purchase of this trip specific additional coverage. In the event you have to cancel, the additional trip insurance coverage could help you receive reimbursement for non-refundable deposits and costs should you have to cancel or interrupt your trip to come home for reasons not covered under the Diver Accident Insurance Coverage.
